In a large bowl, place all the ingredients except the ketchup. Mix the ingredients until well incorporated.
Shape meat into a loaf and wrap in foil or place into a loaf pan that fits your slow cooker.
Arrange the loaf in a 4- to 6-quart slow cooker. Cover and cook on LOW for 6 hours, or until a meat thermometer inserted in the thickest part of the loaf registers 165°F.
